Client Enrollment Process
If you are interested in your child receiving ABA therapy, please complete the online intake form below. You will also find information regarding the intake process and next steps after completing the intake.
The first step in the enrollment process is to complete the online intake form. Please complete this form in its entirety and provide as detailed responses as possible. This helps us best prepare prior to the evaluation.
Prior to scheduling the evaluation, our office will need to receive copies of the following:
- Diagnostic Report (must be a Comprehensive Diagnostic Evaluation completed by an appropriately licensed medical professional)
- Insurance ID card(s)
- Referral from PCP or other licensed professional for ABA therapy services
- Completed and Signed Client Intake Packet)
If you are uncertain whether your child’s diagnostic report meets insurance requirements, or if you need assistance obtaining an appropriate diagnostic evaluation, please contact our office. We are happy to review your documentation and provide guidance throughout the process.
After the online intake has been completed and all required documents have been received, a member of our clinical team will submit an evaluation authorization request to your insurance provider.
Once the authorization request has been approved, a member of our office team will contact you to schedule the in-person evaluation.
Caregivers should expect to complete a comprehensive interview with a member of our clinical team. During this interview caregivers should be prepared to discuss:
- Complete medical history (diagnoses, surgeries, medications, etc.)
- Communication Skills
- Activities of Daily Living and Independent Self-care Tasks
- Social and Interpersonal Skills
- Maladaptive Behaviors and Other Behavioral Concerns
- Current Academic Placement and Functioning (when applicable)
- Caregiver’s Goals for Client/Family
- Scheduling
During the initial caregiver interview, a licensed clinician will typically spend time with your child in our therapy common spaces to complete a skill assessment. This allows us to gain an accurate understanding of your child’s skills and behavioral needs in a peer-based setting, as children often respond differently when parents are present. It also gives caregivers the opportunity to speak openly and comfortably about their child’s development, medical history, and behavioral concerns without their child overhearing sensitive information.
Once the evaluation is complete, please allow 1-3 weeks for a clinician to finalize the initial assessment report and complete all documentation required by you insurance provider for prior authorization. During this time, our team will submit the authorization request on your behalf.
Once authorization is approved, our office manager will contact you to confirm an official start date, answer any remaining questions, and send a welcome email outlining what to expect during your child’s first month of services, including any items to bring or complete prior to starting services.
